The formula is popular in chanting books. I would have thought it occurred much more frequently in the suttas than it does. I found 5 suttas. Anyone know of others?

Get yourself a copy of the CST4 Tipitaka and do a search.

I found these entries for variations of kammassak* in the Suttanta Piṭaka:
  1. 2 times in Uparipaṇṇāsa, Majjhimanikāya
  2. 7 times in Pañcakanipāta, Aṅguttaranikāya
  3. 4 times in Dasakanipāta, Aṅguttaranikāya
  4. 1 time in Jātaka 2, Khuddakanikāya
  5. 2 times in Mahāniddesa, Khuddakanikāya
  6. 2 times in Paṭisambhidāmagga, Khuddakanikāya
  7. 1 time in Nettipakaraṇa, Khuddakanikāya
  8. 1 time in Milindapañha, Khuddakanikāya

Thank you Venerable, I use DPR which searches the same set. The 5 suttas I came up with in my linked blog article are the same ones you've found (minus the KN references).
