once it has been successfully retained it is savitakka ( fixed thought) It appears this is the understanding of some Sri Lankan teachers. But there is not Sutta support for this. At the 4th stage the mind is fixed ( savitakka) on a single object or thought.
The way I understand the Vitakka and Savitakka are the same meaning.

The real question is whether the teaching of Abhidhamma and Visuddhi Magga contain the Buddha's teaching. My understanding is they both contain Buddha's teaching. Will that monk do the same for himself?
Will he die without taking medicine if he got an infection due to worms?
This is not accordance with Buddha's teaching.
